The Geriatric/Long Term Care Nurse Practitioner provides care delivery
and case management and is responsible for establishing and maintaining
effective relationships with patients, patients' families, key
stakeholders, facility staff and collaborating physicians.
In
this vital role, the Nurse Practitioner will work with a dynamic and
engaging team in a collaborative environment at long term care
facilities in the Fox Valley area in Wisconsin.
Education and/or experience:Experience:
Must have 5 years plus experience as Nurse Practitioner. Geriatric or
long term care experience highly preferred. Experience with electronic
medical record preferred.
Education: Graduate from an NLN accredited Nurse Practitioner program.
Licensure/Certification:
Current RN license to practice in the State of Wisconsin and
certification as a Nurse Practitioner for Geriatric, Adult Health, or
Family Nurse Practitioner. Valid DEA.

comprised of one Hospitalist and on Acute Care Nurse Practitioner. This

discharging of internal medicine patients in the Observation Unit as
