Nurse Practitioner/Diabetes
Educator
Mpls/St. Paul, Minnesota
This position has the primary responsibility for
assessing, planning and providing comprehensive diabetes education.
Working with a Diabetes Team consisting of an Endocrinologist and Registered
Dietician, to consult, educate and manage a panel of 500-600 diabetes patients
in an outpatient clinic setting.
Position is based at Aspen's largest clinic at
Bandana Square in St. Paul, MN, with approximately 60 providers, of which
12 are internists.
Requirements
Certified Diabetes Educator (CDE) or willing
to obtain CDE.
Master's Degree in Nursing
A current and unrestricted license for the state
of Minnesota, or able to obtain.
National NP certification, preferably in Adult
or Geriatrics.
Must possess an in-depth and current knowledge
of chronic disease management, diabetes and adult education principles.
Experience with Diabetes education and management.
Excellent presentation and organizational skills
Ability to develop collegial relationships with
patients, physicians and administrators.
